Please follow the steps below to download and complete the order form; then return it to us via fax or U.S. mail. We can ship to most states. 	1. Open and print the order form. Click here to download/open the form. (The form requires Adobe® Reader® software or equivalent.) Then print the form. 	2. Fill out the form. Please print legibly. Credit card orders: we need your credit card billing address in addition to your shipping address.
We can ship to most states in the U.S. Please check the shipping instructions on page 2 of the order form for more information.	3. Figure shipping & tax; total the order. Determine the shipping and tax, as applicable, using the information on page 2 of the order form. Then total your order amount.
